if you have that file and its stylesheet stored locally, please try and out-comment the textfield and see if that helps.
if it doesn't help how about out-commenting the <form>-start and end tags?

I don't think this has anything to do with form tags location.
I think if size=10 is used it should be OK. The question is whether this is a user-friendly size.
