Sorry for the puzzle, you're the first to work on this since Eric Lowe, 
so bear with me while I get you started. Eric, if you have a minute, can 
you answer Mike's question #2 below about the XSL directory and 
Makefile, so he'll know how to proceed? Thanks in advance! My comments

Use the DTD in devref-xml.20070412, this goes for all the source 
documents, I will try to make this more explicit on the template 
information page. Thanks for the feedback.

It should be docbook5-XSL package. Thanks for helping us get to the 
bottom of this...

Hmm, I'm not sure because I'm looking at the 1.70 version of the 
docbook-xsl and it doesn't have an XSL directory either, so I'm stumped 
right now, will dig around some more to jog my memory. We were using a 
tool to generate the HTML for the other books, and I know Eric had this 
book working, but I'll need to check through my histories. I think what 
you suggest is the way to go, and Eric will know for sure. Apologies for 
the delay, I do appreciate your help with this one.

BTW, not sure if you know that this book has been translated via the 
Portals project, so if you can note the changes you make, I can help to 
get them updated in the Japanese version also.
