The engine end of the tacho cable has a square key which slots into the oil 
pump tacho drive. These wear and then start slipping. If only the inner metal 
cable is worn just the cable can be replaced. If the oil pump drive fitting is 
worn it is a pain to fix and at this point you would be considering the 
electric tacho option.
The instrument end can have the same problem with wear...especially if the 
metal inner cable hasn't been pushed all the way into the instrument. Sometimes 
they are just a little to short for the outer sleeve and only engage the 
instrument by a small amount.  If it is the instrument that has worn consider 
moving to a much cheaper electric tacho. I am not sure if the instrument can be 
economically repaired or if the appropriate part is available.
