The main thing is to create the zones, you have to spend 10,000 USD for the 10gig links. This day in time, that is a NOPE, not gona spend that kind of coin. So yeah, most gateways are either 1, 2 or 3 modules single site in the US.

Is it so indispensable that every repeater, that would like to be connected in the World network must be also a gateway and that there is no posibility to use a gateway shared with other repeaters ?
