It is a 3940UW, using Firmware 4.1. Here is a link to the Technical specifications:

As far as I can tell, this card should work fine in all Macs, including G4s. The flash firmware updater Read Me states the following when describing how to update the card from open firmware:
NOTE: If you have a Sawtooth G4 (w/AGP Graphics card - not the 'YIKES' G4-400 motherboard) substitute this line of text in place of the first line above:
dev�/pci@f2000000/pci-bridge/pci-bridge/ADPT,3940UW
The other two lines are the same.
