Many thanks to all who responded.  Gene, N7YW, had a simple solution.
First, change the "SET MODE" command in the  ICOM 746.rig file from "0600FD"
to 060002FD".  Like I said in earlier posts this forces the IC-746 to select
the appropriate mode with a "narrow" (actually "medium") filter.  The second
part is to tell the IC-746 which filters to use for each mode and which
filters to use when the "Filter" or "narrow" mode button is pressed.

I set CW narrow to use the 500 HZ filters and USB/SSB  narrow to use only
the 2.4 filters.

Now when DX4WIN sets the mode to CW, it automatically goes into "narrow"
mode with a 500 Hz filter.  When USB/LSB is selected, the front panel shows
"narrow" mode, but still uses only the 2.4 filters.

It works for me since I don't have or need to use any narrow filters on SSB.
The long-term solution will require Paul to rewrite the rig control code to
allow filter selection.  Perhaps something like CW, CW-N, SSB, SSB-N could
be used to toggle the filters...
