Actually George, when DXbase 2004 was released this past May, the form provided was current with what the ARRL published on their website.
I think it is still valid although their latest preliminary version for LoTW obviously isn't incorporated and won't be until they make a final decision on what they want. Trying to keep up with them is like trying to win a game of poker... you never know what to expect and things constantly change.
