Not sure about the new statistic (I'd vote for a different one!) but I
have traditionally sent this canned email.

Dear Customer,
        In order for you to close your account you will need to do the following
things.

1.      First of all withdraw al the money out of your account, by making an out
exchange, remembering to leave enough behind to cover the out exchange fees.

2.      We will need you to get a notarized form stating that you want us to
close your account with your account number.
3.      After we receive the notarized copy we will contact you from the contact
information you have listed in your account information's history ( so please be
sure to update it if anything has changed)
4.      Lastly we will contact you, and upon verifying we will freeze your
account so no in our out exchanges will be possible.
