We have two identical machines with the same symptoms.
I've attached the lspci output from the second machine (so the MAC
addresses are different from my first post)
FWIW and full disclosure-
According to the yottamark, all 4 cards were manufactured late 2011.

says there was an eeprom change, but seems to indicate that it does not
apply to the F4 cards
ethtool -i eth2 says:
