My favorite is

Bailar, J. C. and Mosteller, F.  eds. (1992). Medical Uses of
  Statistics, 2nd ed. NEJM Books: Boston.  ISBN 0 9101 33360

They give a "balanced treatment of significance testing" according
to Eric Lehmann in a 1997 article.  The book is unfortunately out of
print now but many university libraries will have it.
