Dave, Ok, that makes it even more interesting.
Obviously you have taken a look at things you may have changed since the last time it worked. I think the first thing I would do is a re-boot. If that doesn't correct it take a look at "System Information" in the "About this Mac" menu (the drop down from the Apple logo in the upper left corner.) Does it show the adaptor under the USB menu? I have also had USB/Adaptors fail for no seemingly good reason. Did you get any nearby Lightning or such? Do you have another adaptor to try? First off though, I will be interested in what happens after you re-boot.
