What you're missing is that an antenna and its feedline are a SYSTEM, and antennas can be unbalanced by their surroundings -- ground slope, un-equal mounting heights, conductors in nearby buildings or structure, trees, towers, etc. Any unbalance in the SYSTEM results in common mode current on the feedline.
