This item can be obtained from a number of US sources - such as ED&D - but cheap they are not as we just found out when we boought one!!
A cheaper source is probably your local heavy industrial equipment service organisation - a 2 inch diameter ball bearing is more or less what you need. As for tolerances: if it is a little light or heavy then I am sure a compentent workshop can fix that with a welding torch or a grinder - especially if you get a loop for the cord added at the same time!
