For a new application, the required document per the FCC is listed under
Section 2.1033 of FCC Rules (or 47 CFR 2.1033).  IC has a similar requirement
(RSP-100, RSS-Gen).  Please note IC doesn't accept "-" within the unique
product number.
 
For permissibe change, if you are not the certificate holder, you may have to
change the identifier.  FCC allows changing the FCC identifier and permissive
change being submitted together.  Please refer to FCC KDB 513465, FCC 2.933,
FCC 2.1043.  A similar process applies to IC (multiple listing).
 
In addition to an authorization letter(s), you may need to obtain Schematics,
Block Diagram, and Theory of Operation from the provider.
 
If the changes only apply to the receiver portion of the module and the module
is not a computer peripheral, Class I Permissive Change may apply.  This means
equipment authorization of verification applies.
 
For EU, it is self declaration (learned from members).
