According to sources, a MOV in the base unit can overheat and could lead to a fire. This particular fault would indeed be consistent with the fault only occurring at the upper end of the supply voltage range. If I had to speculate, I would suggest that the MOV’s operating voltage had either been under-specified or a batch of MOV’s are out of spec. Either way, leading them to partially conduct at the upper end of their intended operating range and get hot.
