As per the Guide to the LVD, the voltage limit applies to input and output voltages
Following discussions with Member States, the term "designed for use with a voltage rating" shall be understood as equipment having either a rated input voltage or a rated output voltage (or both) inside this voltage range. Internally there may be higher voltages. The voltage limit applies to the mains power input, not internally-generated or applied voltages. If your adapter doesn't have a mains supply, you have a grey area. I suggest you explain in your safety assessment that it is clearly correct to apply the LVD in such a case, because there is no more appropriate Directive..
