Perhaps it needs a pullup resistor?  What is the resting voltage when its 
all powered up on this index pulse line?  If under a volt on the usual 
digital DVM, I'd try a 4.7k resistor from it to a 3.3 or 5 volt line. 
But have a meter or scope on it when you touch the + line with the 
resistor, and if you don't see pretty close to the + rail, rotate it a 
few degrees and retry it again.  If its stuck down under a volt both 
times, its time to call Heindenhain & see if we don't know what we're 
doing.  There is always that possibility.
