Well, this is a KEY part! There have been a number of operators who really were NOT competent, acting like a nuclear plant was no different from a coal plant, and let things go seriously out of kilter and just kept kicking maintenance down the road. And, the early plants had MUCH slimmer safety margins or ability to deal with breakdowns.

I still think water-cooled reactors just are not a great idea, due to corrosion and the possibility of the water leaking out under high pressure, since the French (and British, too, I think) have demonstrated gas-cooled reactors.
