Haven't heard any follow ups on this thread but I'll go ahead and keep it
going.

After reading the comments and following several threads I've come to
realize that the one big problem we are all facing is a general state of
chaos.  Something like Basecamp, Trac or Redmine would go a long way to
solving these problems.  We should begin searching for a project management
suite immediately.  In my opinion Redmine, is a clear winner.  This should
allow for teams to keep track of projects, issue tickets and whiteboards.
 Then we should have a discourse.gnome.org site to collect feedback from
other teams and the wider community.

The general process should be:

brainstorm (Wiki) --->
setup project (Redmine) --->
discuss (Wiki, IRC, List) --->
execute --->
feedback (Discourse) -->
improve -->
continue...
