The menu item is there, but grayed out.

After turning on the "Online Status" column, I did get the icon and was able
to check "Receive entire message at next connection" (filling in the broken
envelope icon and adding the red arrow). No connection since has resulted in
the attachment being downloaded.

That's a "no" for both : )
This particular account is set to *not* leave copies of messages on the mail
server. Could Entourage be downloading the partial mail message then
blissfully telling the server to delete the full message?
