Really quite baffling! Have you tried sacrificing a chicken?

Let's see, what's left? Have you tried replacing your preference files and
folder (com.microsoft.entourage.plist, and the "Entourage Preferences"
folder)? (Stash the current ones someplace, maybe stuffed; then let
Entourage create new ones.)
