If you look at the new beta version of Virex 7.5, they have real-time "on access" scanning:
http://www.nai.com/us/downloads/beta/virex/ If you don't want to use a beta version, then you could use the Finder's "Folder Actions" to initiate a scan each time something is added to your attachments folder. Look in the AppleScript folder in your Applications folder for more info on Folder Actions.
