If you look at the new beta version of Virex 7.5, they have real-time "on
access" scanning:

If you don't want to use a beta version, then you could use the Finder's
"Folder Actions" to initiate a scan each time something is added to your
attachments folder.  Look in the AppleScript folder in your Applications
folder for more info on Folder Actions.  But the real way to go it seems
would be to use Virex 7.5.
