It sounds like you have a seriously damaged database.

I can only suggest that you try starting a new identity (Entourage menu,
switch identities, 'new') and see if that remains stable. If so, we can
explain how to get whatever information is available out of the old identity
into the new, if npt then it may be you need to remove and re-install office
2004 from scratch.
