So far this has worked for us:
Live Sync OFF
Send commands to server simultaneously OFF
Mark messages as deleted (do not use move)
This has fixed even some really persistent cases of this problem. Although
one person swears that they have still gotten duplicates.
One other data point I would add: For OS X Server, I suspect changing almost
anything related to Mail Services in Server Admin will aggravate this issue,
since Server Admin insists on restarting Cyrus and Postfix for every change,
even if it isn't really needed (e.g., adding members to a mail list).
