It's a bug. It probably fell prey to the Mac convention that a semicolon is considered a word delimiter, not part of a word. AutoCorrect probably only looks at one word at a time. MS Word has its own definitions for words and its own delimiters. Entourage is less advanced, and I guess no one checked this out. (It may also be that the Mac rules changed with OS X and this worked OK back in Entourage 2001 - I doubt it, but it's possible.)
I just added "n;t" replace with "n't" - that also doesn't work . So I see no way around this. I'll report the bug.
However, do note that i you set Preferences/Spelling to "Always check spelling before queuing outgoing messages" and "Always suggest corrections", the spellchecker will always suggest "can't" for "can;t" and so on before it lets you send the message - all you have to do is click "Change" or press Enter (not Return) key. So a message will never go out with the misspelling.

Yes, the option is checked and all other words included in the “Replace With” list seem to work, so long as there’s no semicolon in the incorrect version. They are included on the list, but don’t work. I even tried to add/replace these contractions to the list again to see if that would work.
In your Entourage, if you check that option, does “can;t” change to “can’t”? If so, do you have any idea why only my computers (yes, two of them) won’t correct this error?
And yes I have repaired permissions. OS 10.4.4, Entourage 2004

But you need to have the option "Replace text as you type" checked in that dialog window - in Entourage. Do you? You should also find it already in the "Replace With" list below it? Do you? If not, add it in the Replace and With boxes.
