Ultimately the OWA team at Microsoft steered me toward this KB article:

SYMPTOMS

When you try to open a shared folder that you have delegate access to in Microsoft Entourage 2004 for Mac, you may experience one of the following symptoms:
You receive the following error message:
"The user's folder could not be found. Entourage could not find a mailbox for the user Display_Name"

CAUSE

This issue may occur if the e-mail address that you used to open the shared folder does not match the primary e-mail address for the user who owns the shared folder.

WORKAROUND

To work around this issue, use one of the following methods.
Use the primary e-mail address to open the shared folder
Use the full URL to open the shared folder
Add a secondary SMTP address
 
The first two didn't work; I am trying the third. Not as good as a blanket fix - we will have to add the secondary SMTP address to anyone with a folder that an Entourage user needs to connect...

Have you been able to find the solution to the calendar access problem in Entourage?
 
You need to use the alias / username of the person instead of just selecting them in the address book. If you find them in the address book get the menu up and display the alias, you then go into the advanced button and edit the email address there and enter the alias @ the root domain of the organisation. Although beware that the exchange server alias may be different to the standard username. More information here: http://nelsoncolne.blogspot.com/2006/07/apple-love.html

Rick, it sounds like a DNS problem. Have you tried fully qualified domain names in all your settings? Or how about raw IP addresses of your Exchange Server, that sometimes fixes problems like that.
