Entourage sorts by last name, even though it displays in 'first name, last
name' format. It is possible that the first entry in that list is in the
'first name' field and the last name field is empty (or has a space
character in it). Check how they are set up by opening each contact.

As for the quotes, it is possible that when you copy-pasted info in there
was a comma or some other punctuation in the field - Entourage will
automatically 'protect' such entries with quote characters, as do many
character delimited import routines.
