As far as I know, a camera is doing the following:
1. calculating Flash on the moment of measuring on the point you focus on.
(A-TTL & E-TTL)
2. measuring the Flash on the moment of exposure using the original
point of focus. (= A-TTL, E-TTL & TTL)
So if you recompose in between and your object is out of reach of the used
focus-point then your picture will probably have wrong exposure.
This also depends on the Custom Functions and your camera / flash combo. FEL
should work in this case, but I don't have this feature, so I have no
experience with it.
With my combo (EOS 5 & 430 EZ) I set CF 15, spot-metering and hand-picking
of the focus-point (If I got the time), or just trust on the full automatic,
wich is usually pretty good. (At least much, much better then my old 1000FN
& 430 EZ).
Read your manuals and other information on flash and your combo and try it,
so you can figure out how it works.
