I don't shoot birds all that much, but on a recent trip to Kakadu National Park I shot the 3 with PB-E2, 100-400L with and without 1.4X.
I got about maybe 5% out of focus shots, pretty good going I think. I use CF 17-2, always on centre AF point, AI servo and single shot mode. Focus limiter on 6.5 - infinity, IS off. AF works very well for me. The only problems I found was when the bird flew into a background that was busy - like trees or wavey water. This tends to throw the continuous AF off.
