Hi Andy.
Cannot say for digital bodies, I'm still analog (EOS-3). There was a
long threads on Fred Miranda's EOS board about ETTL (specifically top
gun - 550EX) on digital bodies and many people complained recommending
going "Metz way"...
However, some some others achnowldged excellent performers of 550EX
(ETTL as yours 420EX) on 1D, and I can confirm about my film EOS-3. 
In fact, ETTL is the most precise and reliable system I tried so far (I
used to work with Minolta in some past) producing spot-on results when
coupled to EOS-3 and especially for fill-in. The trick is learning ETTL
system. First of all, any ETTL user MUST understand it's basic
requirement - it's tied to active ASF point for most of its flash
exposure calculation, so each time prior to releasing the shutter, make
sure your current active AF point is right on the subject, preferable
on top of its most improtant part you want to be sure to be exposed
properly (say, people face, forehead). 
Many, even experienced shooters (especially these came from different
systems) do not understand that and then lamenting about severe flash
exposure troubles. 
First fo all I suggest you reading the well laid out EOS (and ETTL
specifically) "bible" that clarifies how  to sue the system in a proper
way to ensure consistant results. The link can be found on my site:

Now, there may be indeed some digital body related issues, cannot say
much about this, though again some 1D users reproted good performers of
the unit.
