As corollary to this, I like to combine the two techniques: put the
flash on the shoe cord and hold it as far to your left as you can, but
with the flash head pointing straight up (clearly this only works in
rooms that don't have extremely high ceilings). This way you get the
modelling from having the light source at an angle to your subject,
but you also get a nice diffuse light from bounce flash, and you get
less wasted light spill than you would by bouncing the light with the
flash on camera.
