Debian GNU/Linux, Exim v4 w/ Exiscan-ACL, SpamAssassin, and ClamAV. I'm very happy with my gateway SMTP server. It took me a few iterations to get it completely tweaked out the way I wanted it, but I've got a very stable and fast setup, that SMTP rejects high-scoring spams and any virus e-mails. E-mails that fall below the rejection threshold are tagged and delivered, where end-user rules usually deliver them to a junk mail folder.
I would think you'll need some type of gateway system, as most antispam products on the market for Exchange don't integrate directly with Exchange 5.5. I've heard good things about the Barracuda Networks Spam Firewall. Two or three schools in the area here use it, and are very happy with it. Relatively low cost, but with an annual fee for maintenance and updates. I believe it's based on SpamAssassin.
