As you are running Exchange 2003, Outlook 2003 and the client has a Windows XP machine then the answer is RPC over HTTPS. You have already opened 443 through the firewall and that is all it needs. Once the initial synchronisation has taken place it will work fine over modem dial up. I have around a dozen users on RPC/HTTPS on dial up at the moment.
