OK. Some progress if anyone is interested. I spent some time on the phone
with MS PSS and it turns out that now, according to the RECENT hosting
whitepapers you are supposed to leave the Authenticated Users in the
Default GAL permissions and make sure to explicitly Deny List Contents to
Authenticated users in the Default GAL permissions.

Of course, about a year ago when I was putting together the hosting
environment, the same whitepaper told me to remove authenticated users
from the Default GAL permissions.

Anyway, I put the Auth Users back and did the explicit Deny and voila -
Outlook 2000 is working for all the jsmith and jdoe users.

BUT... POP3 and IMAP are still having the problem.
