version upgrades can be frightening things

i usually build a new server and migrate the data when i am convinced the new 
server has a chance

often a hardware upgrade is in order and that is not such a big deal but when 
the old hardware is still appropriate and a lot of servers are involved it is 
a big deal

qa should gather a fair amount of feedback about version upgrades in testing

that should be significant in grooming a version before release

inevitably things will be missed

qa should catch those after release

a good faq/howto on those issues would go a long way in miitigating the 
version upgrade problem
