I'd always heard it was a name he assumed on his own, that, is it is not an honorary name. That's why some people who are wise to this fact, and do not recognize him as a "Maharishi", call him by his first name. Perhaps Mr. Varma would be better. The convention of calling him "Mahesh" seems to stem from those who do not accept the grandiose name he assumed. IIRC there is also an interview with one of the Shankaracharya's where they state that it is an assumed name and that he was not a yogi either.

Someone here (Rick?) commented what was actually on his passport, but I cannot remember if it was Mahesh Varma or Srivastava.
