In IIS:
Right click Web sites (to use it for all websites) or the specific website (for only one or for separate setups of rewrite)
Isapi Filters tab
Add...
Browse to the dll
Enter a descriptive name, eg URL Rewriter
OK
Right click the server node
Select All Tasks->Restart IIS...
Choose restart

It's still not working, I'm wondering if it's because I have the
FriendlyURLData servlet running, but I can't figure out how to tell or
where to go to turn it off.

If you are using any other URL rewrite engine you will want to remove
the rewrite servlet -- it will only lead to a world of pain.
