No.  Manuscript format hasn’t changed since Mark Twain.  1” side borders, 1 1/2 
top and bottom, header on each page with author’s last name/one word from the 
title/page number.  12 pt courier.   After the book designer takes hold, I have

Never had that problem, but as I said, I’m outputting vanilla.
