It is not unusual for vets to wrap an animal in a box/plastic bag for the family.  My vets do it and I have had others do it.  It is a matter of people often do not want to see the little one after he has left and secondly there are bodily changes that may be unpleasant for the people to deal with.  Like I said, my vets do this for me then I remove my little friend and put him/her in a box of my choosing for burial. 
 
I am not happy with the vet's refusal to give you copies of the files.  However, I do not find the box to be a problem.  He may well have been trying to do you a favor by fixing Dukee's body for burial. 
 
Did you talk to the vet or a vet tech re getting the records?
 
It is terrible that you are having to deal with this now.  Please do not drive yourself crazy by doubting yourself.

My husband thinks Dukee died the same day he had the surgery and they wanted to make the bucks for having him over the weekend.
He is using some curse words right now. Not a happy camper.
I just told him about my experience with the Vet trying to get copies.
Normally, don't tell him everything unless there is a need.
Didn't want to start anything.
 
I have a question and probably morbid:
 Is there anyway they can find out how long pet has been dead before they are returned back to their owners.
I know they can on humans.
What I think is funny is that he was put in a Science Diet box and it was tape very well. You would have to use a knife to open it. I didn't open the box to look at him really didn't want to see him that way. I wanted to remember as he was. Maybe I should have.....
