Could you have more than one folder named Documents? Have you tried using command-up arrow to navigate upwards from the file to it's folder's enclosing folder's enclosing folder (and so on)?
Andrew, you can use the MP3 tags to reorganize your files. Set a default folder (in itunes prefs) Turn on "keep itunes folder organized" then edit the artist and album tags. The files will be moved into:
<iTunes Default Folder>
artist
albumHave you tried this?
