You guessed it right.  It is 99.9.879% caused by bad installer/install
script.  The worst of the kind is VISE.  Apple provided its installer
SDK, which is OEMed to the famous Zero-G's Installer Anywhere, the
common installer for Solaris.  However, many software vendors have used
VISE for long years, and won't be able to port to Apple Installer
because its script structure is too different.

Any installer which is or before VISE8.02 is a sure way to screw up your
OSX permission.  GPO installer came with my my copy was VISE8.02.  VISE
is now up to VISEX2.1, and seems to be getting better.

Tiger seems to be much better handling of bad installer.  Still, the
standard practice is to run Repair Permission before and after any install.

P.S. Onyx does screw up Permission bit when combined with cron script. 
Last time I checked, it has not been fixed.
