I was able to get the D-side adjusted so that I can close the door without it 
pinching at the top. I used a crow bar to pull the fender forward just enough 
to cram a thin shim uner the fender firewall bolt (Hint: use shims with long 
tabs on this one to have something to hold onto to keep them in place). Also, I 
removed a shim from the lower fender to body bolts near the rocker. This didn't 
raise the fender right at the cowl to fender point but effectively raised the 
fender at the top edge or pinch point. Unfortunately, the large gap is still 
there but at least it's even from top to bottom and I can't get my finger 
through it anymore. I'd love to be able to pull the whole fender back 1/8" to 
get a decent looking gap. I have to wonder if my hinge is goofed. The car has 
been hit on the D-side so it may be possible that the hinge got tweaked or the 
cowl got crushed in just enough to change the swing arc of the door, causing 
this problem. Anyway, now I have to work on pulling the P-side fender forward 
to have enough room to get the windshield molding end clip in place and get the 
gap to look goofy like the D-side ;(
