but it has been broken in more releases than not. Just give up and turn off the smtp protocol fixup.
Better yet, I could suggest you do what we have done. Dump the Cisco PIX and get a CyberGuard firewall. They are incredible and have a much lower TCO. They have no documented vulnerabilities or reported penetrations in the past 5+ years. They are fully Common Criteria EAL4+, ITSEC E3, ... certified.
In fact now they have a deal where one can even trade in an old PIX or other firewall for a $2,000 to $10,000 rebate after one purchases a CyberGuard firewall. Give me a call at 866-742-4777 and I would be happy to tell you about our experience with the CyberGuard products.

It's broken, or Exchange is broken? I've got a few Postfix servers behind
PIX 515UR's, and haven't seen these problems. Exchange boxes (5.5 and
2000) drop messages left and right.
I'd heard (quite awhile back) this was a 'known' issue with Exchange
servers running behind PIXen. Is it known to affect other/any protected
SMTP servers?
