My intitial thought, also, was the bit..A jointed snaffle, with the greater
weight of the driving reins, can fold up and poke her in the palate or
really act like a nutcracker on her tongue, which wouldn't happen so much
while riding.  You might try a solid mullen mouth bit instead to see what
her reaction is.

The lowered head is a clue, as then the weight of the reins would cause the
bit to lie flatter on her tongue, rather than poke her in the palate
