The ah-hoc comes from scope creep & lack of a clear vision, not from
developer planning.  I could plan things to a T, only to have a client
change their mind, and pay for that change, rendering my planning useless..
I don't lose sleep cause I'm getting paid for it.

Seriously, I've quit jobs because of that kind of crap. It drives me insane to write all that code and then have to toss it a few days later when the requirements change drastically. It didn't used to bother me, but now that I know it doesn't have to be that way, I just can't go back.
