If I remember right, it is "off" by default, but there are methods to
programmatically request a rectangular selection.

highlight_rectangular, overlay_rectangular, remove_rectangular,
select_rectangular, etc...

There's no mouse action associated with that - but in your handle method
you could enable that and call the selection functions... (It was a long
time ago, but I think I was calling highlight_rectangular() and it often
seemed to highlight the wrong bits of the display!)

Now, in my test code, this kind of worked, sometimes, with mono-spaced
fonts, but even then was a bit flaky, and was generally useless with any
non-ASCII or non-fixed pitch font...

Indeed.
Though I'm not sure anyone can be using it - we never get anyone asking
about it, and since it was flaky I'm sure we would, if there were users
