Let me start by saying that I am not an expert on MathML. I believe 
that the core issue was: at the time MathML was developing Unicode 
did not have codepoints for its math symbols. I believe that Unicode 
3.2 now has codepoints so a TrueType font could have the MathML 
glyphs defined in the Unicode cmap.
