The region-before/after get printed on blank page unless I specify a conditional which has neither of them. In the piece below, I've got a first page, rest pages and a blank page specified as conditionals. If I've got this wrong, let me know, I'm trying to make things as effecient as possible.

I guess I'm missing something here.  Since you want each page to start
on 1, and also to start on the front of the page, it seems like you can
just set initial-page-number to "1" and force-page-count="end-on-even"
for each fo:page-sequence without all the fo:c-p-m-r's you have below.
