FWIW: Strictly following the Rec, there shouldn't be any objection to missing one or a few column-numbers. If I remember correctly, we implemented it such that FOP 0.91 won't complain unless you explicitly use the missing column-number on a fo:table-cell. When using implicitly numbered cells, it even allows you to have 16 when there's only 15 columns defined...
