It's been a while, but I was able to set it manually within Adobe
Acrobat (not Reader) when I created the bookmark. I was able to
specify something to the effect of expanded (yes | no) via a checkbox
in the add bookmark dialog box. I don't have Adobe Acrobat any more
so I can't follow-up, but I believe this was available from PDF1.3. I
also believe it was 'settable' as a 'default' setting, so that all
were set up this way. In addition, I was able to set up whether or
not the bookmarks window pane was displayed or hidden in a similar
fashion.
